What you should know

International guidelines (including NICE) recommend inpatient treatment of alcohol withdrawal for people at high risk of complications: those with severe withdrawal, a history of seizures or delirium, serious physical illnesses, use of other substances, older age, and those who have not managed to come off a binge at home. In hospital, the doctor can respond to changes in the patient's condition immediately rather than the next day.

Inpatient alcohol detox in Stepnogorsk at Narcology 24 begins with an examination and lab tests, after which an individual treatment plan is drawn up. Medical staff are on duty on the ward around the clock, and an addiction physician assesses progress every day. The patient stays at the clinic voluntarily, on the basis of written informed consent, and can discuss the discharge date with the doctor. The stay is anonymous and does not lead to registration with the state narcology registry.

Who needs inpatient detox rather than treatment at home

  • the binge has lasted more than a week or large amounts of alcohol are being consumed
  • there have been seizures, delirium tremens or severe withdrawal in the past
  • there is heart, liver or pancreatic disease, or diabetes
  • sleeping pills, sedatives or drugs are being taken together with alcohol
  • the person has started hearing or seeing things that are not there or is disoriented
  • there is no adult at home who can stay with the patient during the first 24 hours
  • previous attempts at home detox ended in relapse

If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.

Why severe withdrawal requires a hospital stay

Alcohol withdrawal develops in waves: the first symptoms appear 6–12 hours after the last drink, seizures most often occur within the first 48 hours, and alcoholic delirium on days 2–4. It is not always possible to predict in advance who will develop complications, so it is safer for people at risk to be under constant medical supervision.

In hospital, lab tests are available: a complete blood count, blood chemistry, electrolytes, glucose and, when indicated, an ECG. This makes it possible to detect problems with the liver, kidneys, pancreas or heart rhythm and adjust treatment accordingly. In addition, there is no access to alcohol in a hospital setting, which rules out relapse during the hardest phase of withdrawal.

How admission and inpatient treatment work

Before admission, relatives or the patient can contact the doctor by phone or video to discuss the patient's condition, chronic illnesses, and what belongings and documents to bring. Transport can be arranged if needed. At the clinic, the patient is examined, signs the informed consent form, has blood tests and an ECG, and IV therapy is started. This is followed by daily check-ups, adjustments to the prescriptions, and restoring sleep and nutrition.

An addiction inpatient unit is not a substitute for intensive care. In cases of acute surgical or cardiovascular conditions, bleeding, coma, severe pneumonia or traumatic brain injury, the patient is referred to a general hospital. If the person's life is in danger right now, do not drive them yourself — call 103 or 112.

How we help

01

Medication for withdrawal

Treatment of severe alcohol withdrawal is based on benzodiazepines, which, according to WHO and NICE guidance, reduce the risk of seizures and delirium. Only a doctor prescribes them; the dosage is adjusted using a withdrawal assessment scale (such as CIWA-Ar) and gradually reduced. Taking these medications on your own is unacceptable.

02

IV and vitamin therapy

Intravenous solutions restore fluid and electrolyte balance, and potassium, magnesium and glucose levels are corrected. An essential part of treatment is thiamine to prevent Wernicke's encephalopathy. When indicated, liver-protecting drugs, stomach protection and treatment for any coexisting illnesses that are identified are also given.

03

Round-the-clock monitoring

Nurses regularly measure blood pressure, pulse and temperature, and assess consciousness and the severity of symptoms. At the first signs of delirium or seizures, the doctor changes the treatment plan. This level of monitoring is impossible at home and is the main advantage of inpatient care during the first 72 hours after stopping alcohol.

04

Preparing for the next stage

As the patient stabilizes, a psychotherapist and a psychologist join the treatment: motivational counseling, exploring the causes of relapses and working with the family. Before discharge, the doctor discusses options for staying sober — outpatient follow-up, medication support or a rehab program — and gives written recommendations.

Myths and facts

MythPeople are admitted to an addiction unit against their will

FactAdmission to a private clinic is voluntary and based on the patient's written consent. In Kazakhstan, involuntary treatment is possible only by court order and in the procedure established by law.

MythInpatient care is only for the most advanced cases

FactInpatient care is also recommended when the risk of complications is moderate: with coexisting illnesses, older age or no support at home. It is a way to prevent a severe condition, not just to treat one.

MythAfter discharge, the person will never drink again

FactA hospital stay ensures a safe exit from a binge but does not cure the dependence. Lasting remission requires further treatment: psychotherapy, follow-up with an addiction physician and family support.

Questions and answers

What should I bring to the hospital?

An ID card is not required for anonymous admission, but it is a good idea to bring discharge summaries and a list of current medications. You will also need a change of clothes, slippers, toiletries and a phone charger. Alcohol, medications not approved by the doctor and sharp objects are not allowed.

Can the patient have visitors?

Visits are usually allowed once the patient's condition has stabilized, at times agreed with the attending doctor. In the first few days, contact may be limited to ensure rest and safety. Relatives can receive information about the patient's condition with the patient's consent.

Can the patient leave the hospital early?

A patient receiving voluntary treatment has the right to refuse it. The doctor must explain the possible consequences of early discharge, and the refusal is recorded in writing. If there are signs of acute psychosis or a threat to life, the doctor acts within the framework of the laws of the Republic of Kazakhstan.

How many days does the stay last?

For uncomplicated withdrawal, 3–5 days is usually enough; with a severe course, coexisting illnesses or delirium, the stay is longer. The doctor decides on the length based on changes in symptoms, test results and how the patient feels. Early discharge before the patient is stable increases the risk of complications.

Is a sick note issued?

With anonymous treatment, sick leave certificates are generally not issued, because they require personal details. If you need such a document, discuss it with the clinic administration in advance, and treatment will then be provided on a non-anonymous basis.

How can the family help after discharge?

By supporting the daily routine, removing alcohol from the home, going with the patient to appointments, and avoiding blame or excessive control. A Narcology 24 psychotherapist offers consultations for relatives: how to recognize the signs of relapse and how to talk about difficulties without conflict.

Where do doctors visit in Stepnogorsk and where are the branches?
