What you should know
Sensitizing (aversive) therapy has been used in addiction medicine since the mid-20th century. Its principle is to create a physiological “barrier” to drinking: the drug blocks the enzyme aldehyde dehydrogenase, so when alcohol is consumed, acetaldehyde builds up in the blood. The resulting symptoms — a feeling of heat, facial flushing, a pounding heart, nausea and a drop in blood pressure — act as a deterrent. The method works best in motivated patients who take it under supervision.
Chemical protection against alcohol in Zhanaozen at Narcology 24 is provided by a psychiatrist-narcologist after an in-person consultation and assessment. The patient receives a detailed explanation of the mechanism, how long it lasts, a list of foods and medications containing alcohol that must be avoided, and the consequences of drinking. The procedure is not performed without written informed consent and the required period of sobriety. Chemical protection is seen as support for the decision not to drink, not as a substitute for treating the dependence.
Who chemical protection may be recommended for
- people with alcohol dependence who have made their own decision to stop drinking
- those for whom an external “safety net” matters in the first months of sobriety
- patients after detox who still have a high risk of relapse
- those for whom psychotherapeutic methods have not worked well enough
- people who have stayed sober for the required period and show no signs of withdrawal
- people with normal ECG, blood pressure and liver function
- those who are ready to attend regular consultations with an addiction physician and a psychotherapist
If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.
How the method works and what its limitations are
Normally, the liver converts alcohol into acetaldehyde and then into harmless acetic acid. A sensitizing drug blocks the second step, so acetaldehyde builds up. The reaction can range from moderate (flushing, a pounding heart, nausea) to severe (a drop in blood pressure, heart rhythm disturbances, seizures). That is exactly why the method requires a cardiovascular assessment and strict sobriety.
International guidelines, including NICE, consider sensitizing drugs an option for patients aiming for complete abstinence, provided there is supervision and no contraindications. The weak point of the method is that its effect depends on adherence: if a person is not committed to sobriety, they may stop treatment or take the risk of drinking. That is why chemical protection works best combined with psychotherapy.
How chemical protection works: step by step
Step 1 — a consultation, which can start online: drinking history, length of sobriety, chronic illnesses, medications and allergies. Step 2 — an in-person examination at the clinic: blood pressure, pulse, an ECG and lab tests if needed. Step 3 — choosing the drug and the duration, with detailed guidance on the risks and on foods containing alcohol. Step 4 — signing the informed consent form. Step 5 — administering the drug. Step 6 — scheduled consultations while it is in effect.
Chemical protection is contraindicated during intoxication or without a period of sobriety, and in coronary heart disease, a past heart attack or stroke, arrhythmias, severe high blood pressure, liver or kidney disease with impaired function, epilepsy, psychosis, pregnancy and breastfeeding. If difficulty breathing or chest pain occurs after the procedure, call 103 or 112 immediately.
How we help
Assessment and risk evaluation
Before the procedure, the doctor assesses the condition of the heart, liver, kidneys and nervous system and asks about past illnesses and current medications. An ECG is mandatory, and lab tests are ordered as indicated. If contraindications are found, the doctor offers another method — opioid receptor antagonists, psychotherapeutic coding or treatment without medication-based protection.
Administering the drug
The sensitizing drug is given by intramuscular or intravenous injection, depending on its form. The procedure takes a few minutes, after which the patient stays under observation for a while. The duration of effect depends on the drug and is chosen together with the patient. Only the doctor decides on the dosage and the route of administration.
Safety briefing
The patient receives a list of foods, drinks, cosmetics and medications that may contain alcohol: tinctures, some syrups, kvass, long-fermented kefir and mouthwashes. The patient is told which symptoms indicate a reaction and what to do. Relatives also receive this information if the patient agrees.
Support while it is in effect
Chemical protection buys time during which a person learns to live without alcohol. Narcology 24 recommends using this period for psychotherapy: working through high-risk situations, building skills for managing stress and cravings, and repairing family relationships. Regular consultations help spot the warning signs of an approaching relapse in time.
Myths and facts
MythChemical protection guarantees that the person will never drink again
FactThe drug creates a strong deterrent, but the decision remains with the person. Without psychotherapy and motivation, some patients stop treatment or relapse, so chemical protection is only one part of the program.
MythChemical protection can be slipped into food without the person knowing
FactSecretly giving sensitizing drugs is illegal and life-threatening: a person who is unaware of the risk may drink and have a severe reaction. The method is used only with informed consent.
MythAfter chemical protection you can drink alcohol-free beer
FactAlcohol-free drinks often contain small amounts of alcohol and also remind the person of their familiar drinking ritual. While the drug is in effect, it is recommended to avoid them, as well as other products containing alcohol.

Questions and answers
How long do you need to stay sober before chemical protection?
The doctor sets the sobriety period based on the drug and the patient's health; for sensitizing drugs it is usually at least several days. There must be no alcohol in the body when the drug is given, so a breath alcohol test may be done before the procedure.
How is chemical protection different from Dovzhenko coding?
Chemical protection is a medication-based method: the effect comes from a drug that causes alcohol intolerance. The Dovzhenko method is psychotherapeutic suggestion without medication. They differ in contraindications, mechanism and assessment requirements. The choice is made together with the doctor.
What happens if you drink after chemical protection?
An intolerance reaction occurs: a feeling of heat, facial flushing, a pounding heart, headache, nausea, shortness of breath and a drop in blood pressure. With larger amounts of alcohol, dangerous heart rhythm disturbances and loss of consciousness are possible. In such cases, call 103 or 112 and tell the doctor about the procedure you had.
Can chemical protection be used if you have high blood pressure?
For mild, well-controlled high blood pressure, the decision is made individually after an ECG and a consultation. In severe or poorly controlled high blood pressure, coronary heart disease or arrhythmias, the method is contraindicated. In such cases, the doctor will choose an alternative way to support sobriety.
How long does chemical protection last?
The duration depends on the drug and how it is given — from several weeks to a longer period. The doctor recommends a realistic period that can be extended. Once the drug wears off, it is important to continue treatment so as not to slip back into the old lifestyle.
Is anonymity maintained?
Yes. The procedure is provided on a paid, anonymous basis, with no registration in the state narcology registry. The information is protected by medical confidentiality under the Code of the Republic of Kazakhstan “On Public Health and the Healthcare System.” No medical documents for employers are issued with anonymous treatment.
