What you should know
Chemical protection is chosen when a person has decided not to drink but fears relapsing at a difficult moment, and a surgical implant does not suit or frightens them. It is a form of alcohol “coding” (a treatment widely used in Kazakhstan and CIS countries). For relatives, it is often the first real chance to see their loved one sober for a long time. But it is important to understand that chemical protection is not a “vaccine against alcoholism”: it works only with the person’s consent and together with other kinds of help. Below we explain how the procedure works and how the family can help.
In Astana, we hear from families in which someone works rotational shifts, often travels on business or works a demanding schedule, so regular doctor’s visits are difficult for them. Chemical protection is done in a single visit after preparation and requires no incision. The Narcology 24 doctor sees patients at a clinic that is easy to reach from Yesil, Almaty and Baikonur districts, Saryarka and Nura. Relatives can start with an online consultation.
When chemical protection may suit your loved one
- the person wants to stop drinking and agrees to medication support
- an implant does not suit them because of a fear of incisions or skin characteristics
- there have already been relapses after attempts to quit without a doctor’s help
- they have not been drinking for the required period and are ready for a workup and tests
- there is no serious heart, liver or kidney disease and no mental disorder
- the person is ready to stay in regular contact with the doctor in the first months
- the family is ready to create a sober environment at home and support the decision
If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.
How chemical protection works and why preparation matters
Medications of different groups are used for chemical protection. Some disrupt the metabolism of alcohol (disulfiram-type therapy), so when the person drinks, alcohol breakdown products build up: a hot flush, redness, a racing heart, nausea and a drop in blood pressure appear. Others — opioid receptor antagonists — reduce the craving for alcohol and the pleasure from it. The person knows in advance how the medication works, and this helps them resist the first drink.
Because the reaction to alcohol with sensitizing medications can be dangerous, a medical workup and a period of sobriety are needed before the procedure. Chemical protection right after a binge is unacceptable. Astana’s long winters, with frost and wind, fewer walks and more time at home, are a period of higher risk for many people. It is worth discussing with the doctor in advance how to get through those months sober.
How chemical protection works: steps for the patient and the family
You can start with an online consultation for the family: the doctor will explain how chemical protection differs from an implant and from psychotherapeutic methods and will help you prepare the conversation with your loved one. Then the patient comes for an in-person appointment: a conversation, an examination, tests, an ECG when indicated, and a check of the sobriety period. After the informed consent form is signed, the medication is given intravenously or intramuscularly. The doctor explains the restrictions and stays available for questions.
Chemical protection is not carried out during intoxication or a binge, with serious heart and vascular disease, liver or kidney disease, decompensated diabetes, epilepsy, mental disorders, pregnancy or an allergy to the medication, or without the patient’s consent. If the person drinks after chemical protection and feels chest pain, shortness of breath, a pounding heart or weakness, call 103 or 112 immediately.
How we help
Consultation and medical workup
The doctor finds out how much the person drank and whether there have been binges, seizures or psychoses, as well as illnesses and medications. An examination is carried out, tests are ordered and, if necessary, an ECG. The family can help by telling the doctor about recent episodes the patient may have forgotten. Chemical protection is not done without a workup. The workup does not take long, but it protects against dangerous reactions.
Choosing the medication
The drug group is chosen: a sensitizing medication that causes a reaction to alcohol, or an opioid receptor antagonist that reduces cravings. The choice depends on the person’s health, motivation and readiness to accept restrictions. We do not name doses or brand names: the medication and route of administration are decided by the doctor. The period of effect is also discussed in advance.
Administering the medication
The procedure takes place in a treatment room: intravenously, sometimes as a drip, or intramuscularly. It usually takes 20–60 minutes, after which the patient remains under the doctor’s observation for a while. The same day, the person can go home and return to normal life, but the doctor may advise not driving until the evening.
Support after the procedure
After chemical protection, the Narcology 24 doctor offers consultations with an addiction doctor and a psychotherapist, and meetings with a psychologist for the family. Loved ones should remove alcohol from the home, not drink in front of the person and not “test” the medication’s effect. If strong cravings or thoughts of relapse appear, it is better to discuss this honestly with the doctor in advance.
Myths and facts
MythChemical protection is the same as a “shot that lasts forever”
FactThe medication works for a limited period, and after that the decision to stay sober remains with the person. Chemical protection helps get through the hardest months, but it works better together with psychotherapy and family support.
MythThe medication can be secretly mixed into food
FactThis is life-threatening and illegal. Chemical protection is carried out only with the patient’s knowledge and written consent. A person who does not know about the medication may drink and suffer a severe bodily reaction.
MythTo check whether chemical protection works, you can have a tiny drink
FactSuch a “test” can end in a severe reaction: a drop in blood pressure, heart rhythm disturbances. Do not provoke or encourage such experiments. If the person feels unwell, call 103 or 112.

Questions and answers
How is chemical protection different from an implant?
With chemical protection, the medication is given intravenously or intramuscularly, without an incision. With an implant, the medication is placed under the skin through a small incision. The methods work in similar ways, and the choice depends on the patient’s health and preferences and on the period needed. The doctor will help you compare the options at the consultation.
How long does one need to stay sober before chemical protection?
The period of sobriety is set by the doctor depending on the medication: usually at least several days, sometimes longer. If the person is drinking now, start with detox. Giving the medication while alcohol is still in the body can cause a severe reaction.
Can chemical protection be done at home?
Sometimes — after a workup and if there are no risks — but a clinic is preferable, as it has everything needed for observation after the injection. The doctor makes the decision. If the person is on a binge, detox is done first, and chemical protection later, after a period of sobriety.
How can the family prepare a loved one for chemical protection?
Talk about their own decision, not about your demand. Suggest coming to the consultation together. Do not frighten them with “terrible” consequences — the doctor will explain the risks honestly. Support the period of sobriety before the procedure: remove alcohol and plan weekends without drinking parties.
What should we do if the person drinks anyway?
If chest pain, shortness of breath, a pounding heart, weakness or loss of consciousness appear, call 103 or 112 immediately and say that the person has had chemical protection. If their condition is stable, call the doctor who carried out the procedure. A relapse is a reason to adjust treatment, not to give up on the person.
Is anonymity preserved?
Yes. Chemical protection is provided on a paid anonymous basis without registration with the state dispensary. The information is protected by medical confidentiality under the Code of the Republic of Kazakhstan “On Public Health and the Healthcare System” and is disclosed only in cases expressly provided for by law.
