Psychiatrist home visit in Astana

In short

A psychiatrist home visit is an in-person examination by a psychiatrist in the person's familiar surroundings when they cannot or will not come to the clinic. Relatives can call a Narcology 24 doctor around the clock; the psychiatrist arrives, talks with the patient and the family, assesses the person's condition and risks, gives recommendations or prescribes treatment, and arranges hospitalization if necessary. The visit lasts about 1–1.5 hours and is anonymous.

What you should know

Relatives call a psychiatrist to the home when they realize there is no way to get the person to a clinic. A father hasn't left the apartment for a week and is convinced the neighbors are poisoning him with gas; a grandmother packs her things at night to “go home,” even though she is at home; an adult son, after a day and night without sleep, talks quickly and without pause. At home, the person is calmer, and the doctor sees not only them but also the environment they live in, which allows a more accurate assessment of what is happening.

Narcology 24 psychiatrists make home visits across Astana around the clock — to the Yesil, Saryarka, Almaty, Baikonur, and Nura districts, as well as to Kosshy and Koyandy. In winter this is especially important: driving an anxious or confused person across the city in frost and wind and trying to persuade them in the car is a hard ordeal for both them and the family. The doctor comes to you, and the administrator gives you instructions in advance on how to prepare and what to tell your loved one before the visit.

When Families Should Call a Psychiatrist to the Home

  • the person refuses to leave the house, but their behavior frightens the family
  • ideas of persecution, poisoning, or a “conspiracy” by neighbors or colleagues have appeared
  • an elderly relative mixes up day and night, doesn't recognize family members, and wanders around the apartment
  • there has been almost no sleep for several days, speech is rapid, and spending and actions have become risky
  • your loved one has stopped eating and washing, lies in bed all day, and doesn't respond to anything
  • after discharge from the hospital, their condition is getting worse again, and they refuse to go back
  • you're not sure whether hospitalization is needed and want a doctor to assess this on the spot

If two or three points apply, it is worth seeing a doctor. Phone advice is free and confidential. If life is at risk, call 103 or 112.

Why a Home Examination Is Sometimes More Accurate Than a Clinic Visit

In a doctor's office, many people pull themselves together and try to look “normal,” while at home the things the family sees every day come out. The doctor notices how the person behaves in their own room, whether the windows are covered, whether there are stockpiles of “protective” items, traces of alcohol use, or empty medication packages. These details help distinguish, for example, depression from psychosis, or confusion in an elderly person from a flare-up of a chronic physical illness.

For the family, a home visit is also a chance for everyone who lives there to get answers at once. The psychiatrist explains what is happening, what the risks are right now, how to behave in the evening and at night, what to put out of reach, and when to call 103. For many people who have moved to Astana from other regions and don't have a large family network in the city, it is especially important that the doctor helps build a plan without long trips between institutions.

How a Psychiatrist Home Visit Works

First, you call the administrator or have a video call with the doctor: you describe the situation and give the address, the person's age, and the medications they take. The doctor clarifies the risks in advance and suggests how to prepare the person for the visit without deception or pressure. On site, the psychiatrist introduces themselves, talks with the patient alone and with the family, conducts an examination, and assesses thinking, mood, memory, and risk to life. They then explain their conclusions and discuss the options: monitoring, treatment at home, a follow-up visit, or inpatient care.

A home visit does not replace emergency care. If the person is threatening to kill themselves or others, is holding a knife, has already injured themselves, has lost consciousness, is having seizures, or has severe poisoning, call 103 or 112. Severe psychosis is not treated at home if the patient refuses help and is dangerous: in such cases inpatient care is needed, and involuntary hospitalization is possible only in the manner established by the law of Kazakhstan, by court decision.

How we help

01

Preparing the family for the visit

Before the doctor arrives, it is important not to argue with the person about their ideas and not to promise things that won't happen. You can say honestly: “We're worried about you; a doctor is coming over to talk about your sleep and how you're feeling.” Remove sharp objects, extra medications, and car keys. If you have discharge summaries and the names of medications, put them somewhere visible so the doctor can see them right away.

02

In-person examination and risk assessment

The psychiatrist conducts a conversation and a clinical examination and asks family members about the history of the changes. Special attention is paid to suicidal thoughts, aggression, refusal of food and water, and signs of intoxication or alcohol withdrawal. Afterward, the doctor explains how dangerous the situation is right now, which steps are needed today, and which can be planned for the coming week.

03

Treatment and monitoring at home

If the person's condition allows them to stay at home, the doctor may prescribe therapy and explain how to take it, what to watch for, and when to get in touch again. Medications are chosen individually and only as prescribed by the doctor. Relatives are given clear tasks: monitor medication intake, sleep, and nutrition, and report any changes and warning signs to the doctor.

04

Help with hospitalization

When it is unsafe at home or treatment there is impossible, the Narcology 24 psychiatrist explains to the family why inpatient care is needed and helps arrange voluntary hospitalization. If the person refuses and the risk is high, the doctor will advise where to turn and how to act within the law. The clinic does not take people away by force: all decisions on involuntary treatment are made by a court.

Myths and facts

MythThe psychiatrist will come and immediately take the person to the hospital

FactNo. The doctor comes to examine and help. Hospitalization is discussed if it is dangerous to go without it and is carried out voluntarily; without consent, it happens only by court decision in the manner established by law.

MythIf the person refuses to talk, the visit is pointless

FactEven if the person refuses to talk, the doctor learns a lot from their behavior and surroundings and gets important information from the family. Often the person agrees to talk during the visit itself, once they see a calm professional.

MythThe neighbors will see the psychiatrist arrive, and people at work will find out

FactThe doctor arrives without any identifying markings, and the visit is anonymous, with no registration at the dispensary. Information about the patient is protected by medical confidentiality and is not passed on to the employer. There is no need to tell the neighbors anything.

Questions and answers

What should I tell the person before the doctor arrives?

Speak honestly and calmly: you are worried about their sleep and well-being and have invited a doctor to talk. Don't call it a check-up and don't pretend that a “plumber” is coming — deception destroys trust and gets in the way of treatment in the future.

How quickly does a psychiatrist arrive in Astana?

It depends on the district, the weather, and traffic: during rush hour, getting between the left and right banks takes longer. The administrator will give you an estimated arrival time when you call. If the situation is life-threatening, don't wait — call 103 or 112.

Can I call a psychiatrist for an elderly relative?

Yes, home visits to elderly people with confusion, memory problems, and sleep disturbances are common. The doctor will help distinguish a mental disorder from a complication of a physical illness and, if necessary, recommend tests with other specialists.

Who should be present during the visit?

Ideally, at least one family member who knows the history of the changes well should be there. The doctor will spend part of the conversation with the patient alone. It's better to keep children busy in another room or take them to relatives during this time. That way the doctor can talk calmly with the adults and ask all the important questions.

What should I do if the person is intoxicated?

If they are aggressive, unconscious, vomiting blood, or having seizures, call 103 or 112 immediately. If the person is drinking and also behaving strangely, mention this when you call: an addiction medicine specialist or a psychiatrist experienced in dual diagnosis may be sent on the visit.

What happens after the visit?

You will receive recommendations and a plan: treatment at home, a follow-up visit, online support, or inpatient care. The doctor will explain which signs require an urgent call. A separate session with a psychologist can be arranged for the family to discuss how to support your loved one and yourselves.
