What you should know
When a family first says the word “alcoholism,” there are usually years behind it. First there were holidays and Friday get-togethers, then “unwinding after work,” then hangovers cured with another drink. Relatives hope for a long time that it will pass on its own, especially if the person works, provides for the family, and does not fit the image of “a drinker.” But alcohol dependence is a disease, and the sooner treatment starts, the more health and relationships can be preserved.
In Astana, families often face a particular problem: a heavy workload and the habit of relieving stress with alcohol are seen as normal. A person can hold on to their job for years while everything at home falls apart. Narcology 24 specialists work both with the patient and with loved ones, explaining how to maintain sobriety and what to do in case of a relapse. We do not promise “forever,” but we help build a life in which alcohol is no longer at the center.
Signs of Alcoholism the Family Notices
- the person cannot stop after the first drink and keeps drinking until fully intoxicated
- in the mornings, shaking hands, sweating, and anxiety appear and go away after a drink
- the amount consumed gradually increases, and the usual “limit” no longer has an effect
- drinking becomes more important than family, hobbies, work, and seeing friends
- the person hides bottles, drinks alone, and lies about how much they drink
- memory blackouts appear about the previous evening and where he was
- he keeps drinking despite problems with his health and in the family
If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.
Why Alcoholism Develops — and Why It Matters for the Family to Understand
Alcohol dependence is made up of many factors: heredity, characteristics of the nervous system, chronic stress, anxiety and depression, and the habits of the people around them. It cannot be said that the person is “simply weak-willed.” Alcohol gradually changes how the brain systems responsible for pleasure and self-control work, and giving it up becomes harder and harder. Understanding this helps the family blame less and act more.
Many Astana residents have their own triggers: long, dark winters, frosts, being cut off from family, rotational or shift work, and a corporate culture with obligatory feasts. In winter, intoxication is more dangerous: a person may fall asleep at a bus stop or in a courtyard and suffer hypothermia. It is important for the family to know where the person is and not to leave them alone outdoors in this state.
How Alcoholism Treatment Works
Everything starts with a phone call or an online consultation: the doctor finds out how much and for how long the person has been drinking, whether there have been seizures or hallucinations, and whether there are chronic illnesses. Then comes a home visit by an addiction doctor or hospital admission to relieve withdrawal. After stabilization, tests are carried out, maintenance therapy is selected, and psychotherapy begins. The family receives recommendations at every stage and can take part in separate sessions.
Treatment at home is not possible in cases of delirium tremens or its signs, a history of seizures, serious heart, liver, or kidney disease, decompensated diabetes, or alcohol combined with other substances. In such cases, inpatient care is needed. If there is confusion, hallucinations, seizures, chest pain, vomiting blood, or suicidal thoughts, call 103 or 112 immediately.
How we help
Detox and relief of withdrawal
The doctor administers detoxifying IV solutions, B vitamins, liver-protecting medications, and medications to normalize sleep and reduce anxiety — strictly as prescribed and taking the person’s condition into account. Benzodiazepines are used only by a doctor and only as a short course. Detox removes the acute symptoms but not the addiction itself, which is why the next stage must always follow.
Relapse prevention medication
When indicated, the doctor selects medications that reduce alcohol cravings — such approaches are described in NICE and WHO guidelines. Alcohol “coding” (a treatment widely used in Kazakhstan and CIS countries) and chemical protection (disulfiram-type therapy) are possible only after tests, the required period of sobriety, and written consent. The Dovzhenko method is a psychotherapeutic, suggestion-based technique and likewise requires a voluntary decision.
Psychotherapy
Cognitive behavioral therapy, motivational interviewing, and group sessions help the person understand their triggers and learn to cope with cravings, stress, and loneliness without alcohol. A Narcology 24 psychotherapist works with specific situations: office parties, conflicts, insomnia. This is the foundation of long-term sobriety, without which medications change little.
Family therapy and follow-up support
Loved ones learn to support sobriety without control and reproaches, to recognize the signs of an approaching relapse, and to act calmly. Regular appointments with the doctor after the main course help the person get through the most difficult first months. Online follow-up is convenient for busy people and for relatives who live in other cities.
Myths and facts
MythAlcoholism means drinking every day
FactAddiction can show up as binges with breaks in between or as “civilized” drinking every evening. The key signs are loss of control, cravings, withdrawal, and continued drinking despite the harm. Frequency on its own is not the main criterion.
MythOnce he gets coded, the problem is solved
FactCoding is just one tool, and without psychotherapy and changes in life, the risk of relapse is high. It is carried out only with consent and after tests. The honest approach is comprehensive treatment, not hoping that a single procedure will do the job.
MythIf he holds down a job, it is not alcoholism
FactMany people with addiction keep their jobs and status for a long time. This is called “functional” alcoholism. But over time, health and family suffer, and it is the loved ones who notice the problem first.

Questions and answers
Can alcoholism be treated without the person’s consent?
Voluntary consent is the foundation of treatment. Involuntary treatment is possible only by court decision in the procedure established by the law of the Republic of Kazakhstan. But the family can do a lot to build motivation: a consultation for relatives, a calm conversation, and changes in their own behavior often lead to consent.
How long does treatment take?
Bringing someone out of a binge and relieving withdrawal takes from one to several days. The inpatient stage usually lasts up to two or three weeks. Psychotherapy and follow-up care continue for months. The timeline is individual and depends on how long the person has been drinking, their health, and how involved the family is in the process.
What should the family do after discharge?
Do not turn your home into a prison or watch every step, but do not pretend nothing happened, either. Support a healthy routine, sleep, physical activity, and social contact. Agree on what to do in case of cravings or a relapse. Continue your own sessions with a psychotherapist — this helps you avoid slipping back into old roles.
What should we do if a relapse happens?
A relapse is a common part of the journey, not the end. Do not make a scene, but do not pretend everything is fine, either. Contact the doctor to discuss adjusting the plan. If the binge continues for several days or severe symptoms appear, a home visit by an addiction doctor or a call to 103 is needed.
Do you travel to the suburbs?
Yes, the addiction doctor travels to all districts of Astana on both banks of the Yesil River and to nearby suburbs, including Kosshy and Koyandy. The arrival time depends on the workload and the weather. If there is a threat to life, do not wait for the clinic’s doctor — call 103 or 112.
Can treatment be done if the person has a heart condition?
Yes, but with particular caution. The doctor will assess the risks, do an ECG, and refer the person to a cardiologist if needed. With serious heart disease, it is safer to carry out detox in an inpatient unit under supervision. Be sure to mention all diagnoses and medications when you call.
Where do doctors visit in Astana and where are the branches?
Home visits cover all city districts and nearby settlements, 24/7. Appointments at two branches: 6 Auezova St., office 37, Astana and 176 Imanova St., Astana. Phone in Astana: +7 (700) 309-30-17.
