Alcoholism treatment in Almaty

In short

Alcoholism treatment is a comprehensive program of medical and psychological care for alcohol dependence syndrome: detox, a medical workup, medication support, psychotherapy and rehab. The goal is to stop drinking safely, restore health and maintain sobriety. Narcology 24 provides treatment anonymously, starting with a 24/7 home visit or an inpatient stay; the acute phase takes 3–10 days, and ongoing support lasts for months.

What you should know

For me, alcoholism treatment almost always starts with a call in the middle of the night. The voice on the phone says: “He’s been drinking for ten days, he isn’t eating, he’s shaking in the mornings.” An hour to an hour and a half later I am in the apartment — in Bostandyk district, in a new high-rise in Alatau district, or in a private house in Talgar. I measure blood pressure, check the pupils and the hands, and ask when the last drink was and whether there have ever been seizures. Based on those answers, I decide whether to put in an IV drip at home or take the person to hospital.

But the IV drip is only the first hour of a long journey. By morning the person feels better, and that is exactly when they most often say they can manage on their own from here. My job is to explain that detox relieves the poisoning but does not treat the addiction: the cravings will come back within a few days or weeks. So before I leave, I discuss the next steps with the person and the family: a medical workup, medications to support sobriety, psychotherapy and, if needed, an inpatient stay or rehab.

Signs of Alcohol Dependence

  • a strong urge to drink that is hard to control
  • the person drinks more, and for longer, than they originally intended
  • increasingly larger amounts of alcohol are needed to get the same effect
  • in the morning there is tremor, sweating and anxiety that go away after a drink
  • hobbies, family and work take a back seat to drinking
  • drinking continues despite illness and conflicts
  • attempts to quit end in relapse within a few days or weeks

If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.

Why Alcoholism Develops

Alcohol dependence syndrome (F10.2 in ICD-10, 6C40.2 in ICD-11) develops through a combination of several factors. These include a hereditary predisposition, individual features of metabolism, the habit of relieving stress with a drink, and family and cultural traditions of frequent drinking. Over time, the brain rewires itself: the dopamine reward system starts demanding alcohol, and without it anxiety, irritability and insomnia set in.

Co-occurring mental health conditions play an important role: depression, anxiety, the aftermath of trauma and insomnia. Many people start drinking to numb them and end up in a vicious circle. That is why treatment cannot come down to a single procedure. International guidelines from NICE and the World Health Organization (WHO) recommend combining medical care with psychosocial methods and treatment of co-occurring conditions.

How Alcoholism Treatment Works: From the First Call to Months of Sobriety

The dispatcher will ask about age, how long the binge has lasted, chronic illnesses and any past seizures, and will check whether there are signs that require calling 103. Sometimes the doctor first joins by video. On a home visit, there is an examination, a blood pressure check, an ECG if indicated, an IV drip and 2–4 hours of monitoring, followed by check-in calls in the evening and the next morning. Over the following week come the medical workup, a decision on medication support, the first sessions with a psychotherapist and a plan for the months ahead.

Treatment is not provided at home if there are signs of delirium, seizures, serious heart or liver disease, diabetes with complications, a high fever, a head injury, pregnancy or suicidal thoughts — an inpatient stay is needed, and if there is a threat to life, call 103 or 112. Alcohol “coding” (a treatment widely used in Kazakhstan and CIS countries) and blockers are used only after a workup, the required period of sobriety and written consent, with no promises that it will work “forever.”

How we help

01

Detox and ending a binge

The goal is to stop drinking safely and get through withdrawal. Detoxification IV solutions, B vitamins (especially thiamine, to prevent brain damage), liver-protecting medications and correction of electrolytes are used. Sedatives, including benzodiazepines, are prescribed only by a doctor and under supervision. Moderate and severe withdrawal is treated in the inpatient unit.

02

Medication support for sobriety

After detox, the doctor may suggest medications that reduce cravings or make drinking unpleasant. The choice depends on the condition of the liver, kidneys and heart and on the patient’s goals. Alcohol coding and blockers are also an option, but only after a workup and written informed consent, as part of comprehensive treatment and with no guarantee of results.

03

Psychotherapy

Cognitive behavioral therapy teaches people to recognize the situations, thoughts and emotions that trigger drinking and to replace drinking with other ways of coping. Motivational interviewing strengthens the decision to change. Group therapy and mutual support groups offer support from people with similar experiences. The Narcology 24 psychotherapist also works with the family so that sobriety is supported at home.

04

Rehab and relapse prevention

After the acute phase, it is important to rebuild one’s lifestyle: daily routine, sleep, physical activity, social circle and work. The doctor and the psychotherapist draw up a relapse prevention plan: what to do when cravings are strong, whom to call, how to handle holidays and celebrations. Regular sessions — in person or online — help spot the warning signs of an approaching relapse in time and adjust the treatment.

Myths and facts

MythAlcoholism can be cured with a single procedure

FactNeither an IV drip nor alcohol coding treats addiction on its own. It is a chronic illness, and a lasting result comes from a combination of medical care, psychotherapy, family support and lifestyle change over many months.

MythIf a person only drinks on weekends, it isn’t alcoholism

FactThe diagnosis depends not on a schedule but on the signs: loss of control, cravings, withdrawal and continued drinking despite harm. Weekend binges can also be a sign of dependence and deserve a doctor’s attention.

MythAfter treatment, you can never celebrate anything again

FactSobriety doesn’t cancel holidays, get-togethers or joy. A psychotherapist helps you learn to relax and socialize without alcohol, and a relapse prevention plan prepares you in advance for difficult situations such as weddings and office parties.

Questions and answers

Where do I start with alcoholism treatment?

With a consultation with an addiction doctor — at the clinic, at home or online. If a binge is underway, the first step will be detox. After that come a medical workup and the choice of methods to support sobriety and psychotherapy. It is important not to stop at the IV drip but to agree right away on the next steps and the timeline.

Can alcoholism be treated at home?

Detox for mild to moderate withdrawal can be done at home under a doctor’s supervision. Psychotherapy and medication support can be provided on an outpatient basis. Severe withdrawal, seizures, delirium and serious co-occurring illnesses require inpatient care. The doctor decides this after an examination.

How long does alcoholism treatment take?

The acute phase lasts from a few days to two weeks. It is followed by maintenance treatment and psychotherapy, usually for at least 6–12 months. The timeline is individual: it depends on how long the person has been drinking, their health, motivation and family support. The longer a person stays in the program, the higher the chances of lasting remission.

Is alcohol coding mandatory?

No. Alcohol coding and blockers are just one of the options, and they don’t suit everyone. The decision is made after a workup and a discussion with the doctor. Many patients achieve sobriety through psychotherapy and medications that reduce cravings, without coding.

What should I do if a relapse happens after treatment?

A relapse doesn’t mean the treatment didn’t work. It is a reason to go back to the doctor, look at the causes and adjust the plan. If the relapse has turned into a binge, call a doctor — don’t wait until things get bad. The sooner a relapse is stopped, the fewer the consequences.

Do you treat alcoholism in older adults?

Yes. In older people, withdrawal tends to be more severe, and heart disease, high blood pressure and diabetes are common, so for many of them it is safer to go through detox as an inpatient. Narcology 24 doctors take all current medications into account and choose treatment with particular care.
