Alcohol implant in Almaty

In short

An alcohol implant is the placement of a long-acting medication under the skin that supports giving up alcohol: depending on the substance, drinking either causes a pronounced unpleasant reaction or cravings are reduced. At Narcology 24 in Almaty, an addiction doctor performs the procedure at the clinic after an examination, a period of sobriety and written consent; it takes about 20–40 minutes under local anesthesia. Anonymous, with follow-up care.

What you should know

On house calls, people ask about an implant almost as often as about an IV drip. Most often it is relatives: “Let’s get him an implant before he starts again.” I explain why time must pass between an IV drip in the apartment and an implant. If there is alcohol in the body, the implant medication can cause a severe reaction. So first comes getting out of the binge, then sobriety, an examination and tests, and only then the procedure. In Kazakhstan, the implant is one of the forms of alcohol “coding” (a treatment widely used in Kazakhstan and CIS countries).

The procedure itself takes place at the clinic, not in a kitchen. The person comes, for example, from the Bostandyk district or from Kaskelen, ideally in the morning to avoid evening traffic on the roads out of the city. Before the procedure: a repeat examination, blood pressure and, if needed, an ECG and blood tests, and signing the consent form. The placement itself takes about half an hour, after which the person stays under observation for a while and goes home the same day.

When a Doctor May Suggest an Alcohol Implant

  • the person has decided to stop drinking and wants reliable external support for months
  • there have been relapses after IV drips and other methods in the first weeks of sobriety
  • there is a stable period of sobriety and no signs of withdrawal
  • tests have revealed no contraindications to the procedure
  • it is hard for the person to take pills every day on schedule
  • the person is ready to see a psychotherapist and an addiction doctor at the same time
  • loved ones understand the risks of drinking alcohol with an implant

If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.

How the Implant Works and Why Preparation Is Needed

Long-acting medications are used for implants. Some of them interfere with the breakdown of alcohol, so when a person drinks, substances build up that cause flushing, a pounding heart, nausea and a drop in blood pressure. Others are opioid receptor antagonists that reduce the pleasure from alcohol and cravings for it. The doctor decides which option is suitable, taking into account the person’s health and history of addiction. The mechanism is always explained before the procedure.

Preparation is a matter of safety. Alcohol in the body at the time of implantation or shortly afterward can trigger a severe reaction, up to and including a threat to life. Contraindications include heart and vascular disease, serious liver and kidney disease, a number of mental disorders and pregnancy. That is why an implant without an examination, “at the relatives’ request,” or without the person’s knowledge is unacceptable.

How an Alcohol Implant Is Placed: Preparation, Procedure, Follow-Up

You can start with a video consultation with an addiction doctor to discuss the history of drinking, previous treatments, illnesses and expectations. Then comes an in-person appointment, confirmation of the period of sobriety, an examination, an ECG and blood tests as indicated, and a written informed consent form. In the treatment room, the skin is numbed with a local anesthetic, a small incision is made, the implant is inserted, and a stitch or dressing is applied. Afterward: observation, advice on caring for the insertion site and a follow-up visit.

An implant is not placed if the person is intoxicated or has not been sober long enough, or with acute infections, serious heart, liver or kidney disease, certain mental disorders, pregnancy and breastfeeding, an allergy to components of the medication, or without the person’s consent. If redness, swelling or pus appears at the insertion site, see a doctor. If the person has had a drink and feels unwell, call 103 or 112 immediately.

How we help

01

Examination before implantation

The doctor takes a medical history, confirms the period of sobriety, measures blood pressure and pulse and, if necessary, orders an ECG and a blood chemistry panel. Special attention is paid to the heart and liver, because they are the organs that suffer most in a reaction to alcohol. If contraindications are found, we discuss alternatives: psychotherapeutic methods or oral medications under supervision.

02

The implantation procedure

It is carried out in the treatment room under sterile conditions. The insertion site is usually below the shoulder blade, in the buttock or another area that heals well. Local anesthesia, a small incision, insertion of the implant, and a stitch or sterile dressing. The procedure is quick and causes little pain, and afterward the person can return to their normal routine with a few minor restrictions.

03

Aftercare and follow-up

For the first few days, the insertion site must not get wet or be rubbed, and it is advisable to avoid strenuous exercise, steam baths and saunas. We agree on a follow-up examination. If redness, tenderness or a fever appears, it is important to see a doctor right away. As a rule, healing takes place without complications within a few days.

04

Psychotherapy while the implant is active

An implant buys time, but it does not change habits or ways of coping with stress. That is why we recommend sessions with a psychotherapist, work with the family and, if necessary, treatment for anxiety and insomnia. As the end of the period approaches, we discuss how to stay sober going forward, so that the end of the implant’s effect does not become a trigger for relapse.

Myths and facts

MythAn implant works forever

FactAn implant works for a limited period, after which the medication is fully absorbed. Its purpose is to help the person through the most vulnerable period of sobriety. After that, much depends on what changes the person has managed to make together with the doctor and psychotherapist.

MythYou can give someone an implant while they are asleep

FactPlacing an implant without the patient’s knowledge and consent is unacceptable, dangerous to their life and against the law. We perform the procedure only on a sober person, after an examination and the signing of a written informed consent form.

MythYou can just “dig out” the implant and drink

FactRemoving an implant yourself carries a risk of infection and bleeding, and by then part of the medication may already be in the bloodstream. If the person wants to stop using this method, it is better to discuss it with a doctor rather than take risks.

Questions and answers

How many days do you need to stay sober before an implant?

The required period of sobriety depends on the medication and the person’s health and is determined by the doctor. It usually means several days or more. Before the procedure, the time of the last drink is always checked, and if there is any doubt, the procedure is postponed — this is a matter of safety, not a formality.

Does getting an implant hurt?

The procedure is performed under local anesthesia, so most people feel only mild discomfort. For a few days, the insertion site may be slightly tender. Severe pain, swelling or redness are reasons to see a doctor, because they may be signs of inflammation.

Can an implant be placed at home?

We perform implantation at the clinic, which has a treatment room and sterile conditions. A house call is possible at the preparation stage: a doctor from the Narcology 24 mobile team will help with getting out of a binge in any district of Almaty or the suburbs, and then you come in for the procedure.

What happens if you drink with an implant?

It depends on the medication. With some, a severe reaction occurs with a pounding heart, a drop in blood pressure, vomiting and difficulty breathing — a reason to call 103 or 112 immediately. With others, alcohol simply stops giving the usual pleasure. The doctor will tell you in advance about the specifics of your implant.

Can the implant be removed early?

Yes, if there are medical reasons or at the patient’s request, the doctor can remove the implant in the treatment room. You must not do this yourself. Before removal, we usually discuss what has changed and how to stay sober going forward, so that the decision does not become a trigger for relapse.

Is an implant anonymous?

Yes, the procedure is carried out anonymously, without state dispensary registration. Information is protected by medical confidentiality under the Code of the Republic of Kazakhstan “On Public Health and the Healthcare System” and is disclosed only in cases expressly provided for by law. The insertion site is chosen so that it is not noticeable.
