Binge drinking treatment in Almaty

In short

Binge-pattern alcoholism is a form of alcohol dependence in which periods of continuous heavy drinking lasting several days alternate with sober intervals. Treatment includes ending the binge under medical supervision, detox, restoring organ function and a program to prevent new binges. Narcology 24 provides home visits to stop a binge 24/7; the acute phase usually takes 1–5 days, after which relapse prevention treatment begins.

What you should know

Binge calls follow the calendar: after long holidays, in early January and after Nauryz, we work almost without a break. The pattern is nearly always the same. A person doesn’t drink at all for a month or two, then has one drink at a birthday party — and a week later his wife calls at three in the morning: he can’t stop, he can’t eat, his hands are shaking all over. On empty night roads we get to Turksib district quickly; during the day, in traffic, it takes longer.

In the apartment, I see the classic picture: bottles and blood pressure pills on the table, a man sitting on the edge of the bed, nauseous, sweating and afraid to be left alone. He says he wants to “come off it the right way.” I examine him, check for signs of developing delirium, set up an IV drip and stay until I am sure his blood pressure and pulse have stabilized. But the most important conversation is about how to prevent the next binge.

Signs of Binge-Pattern Alcoholism

  • drinking goes on for several days in a row, from morning till night
  • after the first drink, it is impossible to stop until exhaustion sets in
  • between binges, there are weeks or months of complete sobriety
  • toward the end of a binge, appetite disappears and vomiting and insomnia appear
  • trying to stop abruptly causes tremor, fear and a racing heart
  • binges happen more often, and the sober intervals get shorter
  • after a binge, the person is left with guilt, memory blackouts, debts and conflicts

If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.

Why Binges Happen

A binge pattern of drinking usually indicates a middle or late stage of dependence. Control over the amount has been lost: the first drink sets off a powerful craving, and the body can no longer stop without help. The longer a binge lasts, the greater the changes in the nervous system and the more severe the withdrawal. A separate condition is dipsomania — periodic episodes of binge drinking that are sometimes linked to mood disorders.

In many people, a binge is preceded by changes in mood: low spirits, irritability, insomnia and inner tension. This is an important signal that can help prevent a relapse. Stress, holidays, conflicts, payday and loneliness are the most common triggers. If they aren’t addressed, the sober intervals remain merely pauses between binges rather than real remission.

How Ending a Binge and Treating Binge-Pattern Alcoholism Works

When you call, the dispatcher will ask how many days the binge has lasted, when the last drink was, and whether there is vomiting, seizures, hallucinations or chronic illness. The team usually arrives within an hour to an hour and a half. The doctor examines the person, sets up an IV drip and monitors them for 2–4 hours. The next day we check in by phone and repeat the visit if needed. On the third to fifth day, we discuss a medical workup and a program to prevent new binges.

A binge must not be ended at home if there are signs of delirium, seizures, a serious injury, a temperature above 38 °C (100.4 °F), dehydration with inability to drink, serious heart or liver disease, or pregnancy. If the binge has lasted more than two weeks or there has been delirium in the past, an inpatient stay is safer. If there are hallucinations, confusion, seizures or thoughts of death, call 103 or 112.

How we help

01

Ending a binge under medical supervision

Stopping drinking abruptly after a long binge is dangerous: seizures and delirium can occur on the second to fourth day. That is why the process is supervised: IV drips with detoxification solutions, B vitamins, correction of electrolytes and symptomatic care. Sedatives are used only when prescribed by a doctor. This makes it possible to get through withdrawal more safely and more easily.

02

Recovery after a binge

After many days of heavy drinking, the liver, pancreas, heart and nervous system all suffer. The doctor orders a medical workup, liver-protecting medications and vitamins as indicated, and gives advice on diet and sleep. If insomnia or anxiety appears, safe therapy is selected. Recovery takes weeks, and during this period the risk of returning to alcohol is especially high.

03

Relapse prevention treatment

To keep a sober interval from becoming just a pause before the next binge, the doctor offers medications that reduce cravings, or alcohol “coding” (a treatment widely used in Kazakhstan and CIS countries) and blockers — only after a workup, a period of sobriety and written consent. Co-occurring depression, anxiety and sleep problems, which often trigger the next relapse, are treated separately. A psychiatrist is brought in if needed.

04

Working with early warning signs of relapse

The psychotherapist teaches the person to recognize early signals: mood changes, irritability, thoughts like “just one won’t hurt,” insomnia. A personal plan is drawn up: what to do at the first signals, whom to call, when to contact the doctor. The family knows this plan too. Regular sessions at Narcology 24 help lengthen the sober periods and make binges rarer and shorter.

Myths and facts

MythYou can come off a binge on your own just by stopping drinking

FactAfter a long binge, abrupt withdrawal can cause seizures and alcohol withdrawal delirium (delirium tremens). It is safer to come off under a doctor’s supervision, especially if there has been severe withdrawal in the past or there is heart disease.

MythIf a person doesn’t drink between binges, they aren’t ill

FactLong sober intervals are typical of the binge form of dependence. This isn’t remission but part of the illness. Without treatment, binges usually become more frequent, longer and harder on the body.

MythAn IV drip completely solves the problem of binges

FactAn IV drip relieves intoxication and eases withdrawal, but it doesn’t remove cravings. Without relapse prevention treatment and psychotherapy, the next binge often happens within just a few weeks.

Questions and answers

How long does it take to come off a binge?

For a short binge, one visit and a day of recovery. For a long one, two to five days with repeat visits or in the inpatient unit. Full recovery of sleep, appetite and well-being takes several weeks. The doctor will assess the condition and tell you what to expect in your particular case.

Can I call a doctor at night to stop a binge?

Yes, the team works around the clock, including holidays. At night, the drive across the city takes less time. If you are waiting for the doctor and the person’s condition gets worse — seizures, hallucinations, or they don’t recognize their family — don’t wait: call 103 or 112.

What should I do before the doctor arrives?

Don’t give alcohol “to take the edge off” or sleeping pills that weren’t prescribed. Offer water in small sips, and lay the person on their side if they feel sick. Get together a list of the medications they take and a blood pressure monitor. Don’t leave them alone.

Is a hospital stay necessary for a binge?

Not always. For many people, treatment at home is enough. An inpatient stay is needed for a very long binge, severe withdrawal, past delirium or seizures, serious illnesses, or when there is no one who can stay with the person. The doctor makes the decision after an examination.

How can the sober periods between binges be extended?

Relapse prevention treatment is needed: medications that reduce cravings, psychotherapy, treatment of co-occurring depression and insomnia, and work on triggers. An action plan for the first signs of an approaching binge is also important. Narcology 24 doctors follow patients for months, including online.

Do you make home visits to the Almaty suburbs?

Yes, the team travels to Kaskelen, Talgar, Konaev and other towns and villages near the city. The trip out of town takes longer, so it is better to call early, as soon as it becomes clear that the person can’t stop.

Where do doctors visit in Almaty and where are the branches?
