Delirium tremens: emergency help in Almaty

In short

Delirium tremens (alcohol withdrawal delirium) is an acute psychotic state that usually develops 2–4 days after stopping long-term heavy drinking: confusion, hallucinations, fear, tremor and fever. It is a life-threatening condition and is treated only in hospital. If you suspect it, call 103 or 112. Narcology 24 in Almaty helps you recognize the first signs, prevent delirium during alcohol detox and arrange hospitalization.

What you should know

I usually learn about alcohol withdrawal delirium from a single phrase from relatives: “He’s talking to someone who isn’t there.” The call comes at night, most often on the third day after the person stopped drinking. He is not sleeping, he is pacing around the apartment, he sees insects and people outside the window, and he is frightened. In this situation, the dispatcher does not promise a quick visit with an IV drip but says right away: call 103 or 112, this condition is treated in hospital, and we will help if the ambulance is delayed.

Even when our team is the first to arrive — in the Turksib district or in the private-house neighborhoods of Talgar — the doctor’s job is not to “cure” the delirium at home. We assess breathing, pulse, temperature and blood sugar, ensure the safety of the person and those around them, give stabilizing medications if needed, and arrange hospitalization. Delirium can be complicated by seizures, heart rhythm disturbances and brain swelling, and round-the-clock monitoring is required.

Early Signs of Delirium Tremens

  • insomnia and growing anxiety and restlessness 1–3 days after stopping drinking
  • severe trembling of the hands and whole body, sweating, a rapid pulse
  • the person does not understand where they are and confuses the time of day and people
  • visual hallucinations: insects, animals, “people” in the room
  • fear, a sense of being pursued, attempts to run away or hide
  • raised body temperature, high blood pressure, a pounding heart
  • a seizure before confusion sets in

If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.

Why Alcohol Withdrawal Delirium Develops

Delirium is a severe form of alcohol withdrawal syndrome. After many years of regular drinking, and especially after a long binge, the nervous system is thrown into a state of extreme overexcitement when alcohol is suddenly stopped. It usually starts 48–72 hours after the last drink, sometimes later. The risk is higher in people who have already had delirium or seizures, in older people, and with liver disease, infections or head injuries.

Delirium is often mistaken for “ordinary” drunkenness or a hangover, and time is lost as a result. The difference is that the person is sober, but their mind is confused, they see and hear things that are not there, and they cannot critically assess what is happening. Without treatment, delirium is life-threatening. That is why it is much more important to prevent it: with long binges and a history of delirium, alcohol detox is best done in an inpatient unit.

What to Do If You Suspect Delirium: Step by Step

First, call 103 or 112 and say plainly that you suspect alcohol withdrawal delirium. Second, do not leave the person alone; remove sharp objects, close the windows and the balcony, and speak calmly and briefly. Do not give alcohol or sedatives from the home medicine cabinet. While the ambulance is on its way, you can call Narcology 24: a doctor will advise you by phone or video on what to do and, if needed, arrange admission to our inpatient unit after an examination.

An online consultation cannot replace care for delirium: by video it is impossible to measure temperature, blood pressure and oxygen saturation or to assess the risk of seizures. Home treatment is not possible in any form. In severe cases, with high fever, seizures or breathing problems, a general hospital with an intensive care unit is needed. If the person threatens themselves or others, call 102 or 112 — your safety comes first.

How we help

01

Stabilizing the condition

In the inpatient unit, vital signs are assessed first: breathing, pulse, blood pressure, temperature, blood sugar and electrolytes. The patient’s safety is ensured, and with marked agitation, medical sedation is given as prescribed by the doctor. The goal of the first hours is to reduce the strain on the heart and nervous system and to prevent seizures.

02

Medication treatment

According to the clinical protocols of the Ministry of Health of the Republic of Kazakhstan and international recommendations, including NICE, the mainstay of treatment for withdrawal delirium is benzodiazepines, which are selected and titrated under a doctor’s supervision. Other medications are added if necessary. Treatment is given only in an inpatient setting, where breathing and consciousness can be monitored continuously.

03

Vitamins and metabolic correction

A mandatory part of treatment is giving thiamine and other B vitamins to prevent Wernicke’s encephalopathy, replacing fluids and electrolytes, and correcting blood sugar levels. At the same time, doctors look for and treat coexisting conditions — infections, injuries, liver and pancreatic damage — that can make delirium worse.

04

Preventing recurrence

After recovery from psychosis, the next stage begins: a conversation with an addiction doctor and a psychotherapist, an assessment of the severity of the addiction and a treatment plan. A person who has had delirium is at high risk: with another binge, delirium may recur. That is why we strongly recommend follow-up addiction treatment and detox only in an inpatient unit.

Myths and facts

MythDelirium tremens happens when a person is drunk

FactDelirium develops not during intoxication but after drinking stops, usually on days 2–4 of sobriety. That is exactly why it is easy to miss: relatives think “the worst is over,” when in fact it is only just beginning.

MythYou can wait it out at home; it will pass on its own

FactDelirium sometimes resolves on its own, but without treatment it can end in seizures, heart rhythm disturbances and death. Waiting is dangerous. At the first signs, call 103 or 112. Do not wait until morning.

MythGiving a little drink will make things easier

FactAlcohol may temporarily mask the symptoms, but it does not treat delirium; it only delays it and makes it more severe, and with confusion it also creates a risk of aspiration. This is not help but additional harm.

Questions and answers

How many days after a binge can delirium tremens start?

Most often 48–72 hours after the last drink, sometimes on days 4–5. Warning signs — insomnia, growing anxiety, severe tremor — may appear earlier. If you see them in someone after a long binge, it is best to seek medical help right away.

Can delirium tremens be treated at home?

No. Alcohol withdrawal delirium is treated only in an inpatient unit with round-the-clock monitoring, and in severe cases in intensive care. A doctor’s house call can help assess the situation and arrange hospitalization, but it does not replace inpatient treatment. If you suspect delirium, call 103 or 112.

How can delirium be prevented during alcohol detox?

Do not abruptly stop a long binge without medical supervision, especially if delirium or seizures have occurred before. In such cases, detox is best done in an inpatient unit, where the doctor will prescribe preventive treatment. We will help you choose a safe option after an examination or a video consultation.

How long does treatment take?

The acute period of delirium usually lasts 3–5 days, sometimes longer. After recovery from psychosis, several more days of observation and recovery are needed, especially with coexisting illnesses. Then we recommend addiction treatment, because with another binge the risk of recurrence is high.

Can someone be hospitalized without consent?

A person in delirium often cannot assess their own condition. In an emergency, act through 103 or 112. Planned compulsory treatment for addiction is possible only by a court decision in accordance with the law of the Republic of Kazakhstan; a private clinic treats patients only on a voluntary basis.

Does Narcology 24 help with hospitalization?

Yes. The on-duty doctor will tell you by phone what to do until the ambulance arrives and, if the person’s condition allows, arrange transport to our inpatient unit from any district of Almaty and the suburbs. In severe cases, the priority is a general hospital with an intensive care unit.

Where do doctors visit in Almaty and where are the branches?
