What you should know

If, after a binge, your loved one has started having “visions,” is talking to people who are not there, does not recognize you or does not understand where they are, this may be delirium tremens. It is a medical emergency in which, without treatment, serious complications and death are possible. The most important thing to do right now is to call 103 or 112. Below we explain how to behave until the doctors arrive, what you must not do, and how to prevent delirium in the future.

Delirium most often begins on days 2–4 after a long binge is stopped abruptly, frequently in the evening or at night. In Astana, families sometimes mistake the first signs for ordinary insomnia or “nerves” and wait until morning. Winter is especially dangerous: a confused person may go out into the frost and wind without realizing where they are. Our specialists advise relatives and treat delirium in an inpatient setting.

Signs of delirium tremens the family may notice

  • the person sees things that are not there: insects, animals, people, “threads” on their skin
  • they do not know where they are, confuse day and night, and do not recognize loved ones
  • severe shaking, heavy sweating, a rapid pulse, a raised temperature
  • sudden fear and anxiety, a feeling that they are being followed or that someone wants to harm them
  • they have not slept for several nights in a row, are restless and talk to someone who is not there
  • seizures or brief loss of consciousness after stopping a binge
  • they try to run away, climb out of a window or go outside without clothes

If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.

Why delirium tremens happens and who is at risk

Alcohol withdrawal delirium is a severe form of withdrawal syndrome. It develops when alcohol is stopped abruptly after prolonged, heavy drinking and the nervous system cannot readjust. The risk is higher with many years of addiction, long binges, previous seizures or delirium, liver disease, exhaustion, head injuries and infections. Delirium does not come “from a single drink”: it is the result of long-term strain on the body.

Many families in Astana face a situation where the person decided to “quit” on their own, stopped abruptly, and a couple of days later strange things began to happen. That is why, after a long binge, it is safer to stop under a doctor’s supervision. If your loved one has had delirium tremens before, the risk of it recurring during the next binge is higher — it is important to plan inpatient care right away rather than wait for symptoms.

What the family should do and how delirium is treated

Call 103 or 112 immediately and say that the person has hallucinations and confusion after a binge. Until the doctors arrive, speak calmly and briefly, do not argue with the “visions,” remove sharp objects, close the windows and balcony doors, and do not give alcohol or sleeping pills. After stabilization, the inpatient unit provides IV drips, withdrawal treatment, and monitoring of the heart and breathing. By phone, we advise relatives on what to do.

Delirium cannot be treated at home, by phone or online: it requires round-the-clock observation and access to intensive care. Do not try to “bring the person out of it” with alcohol — this is not treatment and only postpones the problem. If the person is aggressive and dangerous to themselves or to you, move to a safe place and wait for 103 or 112. We do not take anyone away by force: if necessary, the matter is resolved in the manner established by the law of the Republic of Kazakhstan.

How we help

01

Emergency stabilization

The first task is to ensure safety: monitoring breathing, blood pressure, pulse and temperature and, if necessary, stopping seizures. This is done by the ambulance crew and then by the inpatient doctors. It is important for relatives to tell them how long the binge lasted, when the person last drank, what illnesses and medications they have, and whether there have been similar episodes before.

02

Inpatient treatment

Treatment includes IV drips to correct dehydration and electrolytes, B vitamins to protect the nervous system, and medications to reduce agitation and prevent seizures, including benzodiazepines as prescribed by the doctor. Treatment is given under constant observation, because the condition can change within hours.

03

Prevention during the next binge

If the family has already experienced an episode of delirium, do not wait for symptoms to appear during the next binge. Call the doctor in advance: if the risk is high, it is better to end the binge under supervision in the Narcology 24 inpatient unit. Relatives should write down the circumstances in which delirium developed last time — this will help the doctor.

04

Family support after delirium

After recovering from psychosis, the person often feels shame and fear, and relatives feel confused and angry. A psychologist helps talk through what happened and agree on rules and further addiction treatment. It is important that relatives get support for themselves too — a loved one’s delirium is a hard ordeal for the whole family.

Myths and facts

MythDelirium tremens starts when a person is very drunk

FactOn the contrary: delirium usually develops 2–4 days after a long binge ends, when the person is already sober. That is why stopping a binge abruptly without a doctor is dangerous for people with long-standing addiction.

MythDelirium will pass on its own if the person sleeps it off

FactWithout treatment, delirium can lead to serious heart and brain complications and to death. You must not wait for the person to “sleep it off.” At the first signs, call 103 or 112 immediately.

MythIf you give them a little to drink, the “visions” will go away

FactAlcohol may temporarily ease the symptoms, but it does not treat delirium and worsens the outlook. This is a dangerous folk practice. The person needs medical care in an inpatient unit, not another dose of alcohol.

Questions and answers

How can I tell delirium from an ordinary hangover?

With a hangover, the person clearly understands where they are and recognizes loved ones, and the unpleasant feelings are limited to weakness and nausea. With delirium, hallucinations, disorientation, intense fear and impaired consciousness appear. If you are in doubt, call 103 or 112 — it is better to be safe than sorry.

How should I talk to the person during delirium?

Briefly, calmly and slowly. Say who you are and where they are: “I’m Aigerim, your wife, you’re at home.” Do not argue with the hallucinations or laugh at them. Avoid sudden movements. If they are aggressive, keep your distance, take the children away and wait for the ambulance.

What must not be done before the doctors arrive?

Do not give alcohol, sleeping pills or sedatives without a prescription, do not tie the person up, do not lock them alone in a room, and do not leave windows or the balcony open. Do not let them go outside, especially in winter in Astana, when the frost and wind are dangerous. Wait for 103 or 112.

Can delirium tremens be treated at home?

No. Delirium requires an inpatient unit with round-the-clock observation and access to emergency care. At home, it is impossible to monitor the heart and breathing or to change treatment quickly. Even if the person seems to feel better, their condition can deteriorate sharply.

Will delirium happen again?

If the binges continue, the risk of another episode of delirium is higher, and each episode is harder on the brain and heart. That is why, after discharge, it is important to treat the addiction itself: psychotherapy, a doctor’s support, alcohol “coding” (a treatment widely used in Kazakhstan and CIS countries) after a medical workup, and rehab. The family can start with a consultation with an addiction doctor.

How can relatives cope with what they have been through?

Seeing a loved one in delirium is frightening, and it often leaves anxiety, insomnia and anger behind. This is a normal reaction. A family consultation with a Narcology 24 psychologist, including online, helps you come to terms with what happened and agree on next steps without mutual blame.
