Alcohol coding in Astana

In short

Alcohol “coding” (a treatment widely used in Kazakhstan and CIS countries) is a group of medication-based and psychotherapeutic methods that help a person with alcohol dependence stay sober for a set period. It is carried out voluntarily, after a medical workup, the required period of sobriety and written informed consent. For the family, it is a way to support a loved one’s decision. At Narcology 24 in Astana, coding is part of comprehensive treatment; it is anonymous, and the duration is chosen with the doctor.

What you should know

Many relatives call with the question: “How can I get my husband (father, son) coded?” We tell them honestly right away: coding is impossible without the person’s own consent. It is not a punishment or a way to “force” someone, but a tool that helps those who have decided themselves to stop drinking. Here we explain what methods exist, how to prepare your loved one, what the family can and cannot do, and why coding works better together with psychotherapy.

In Astana, people often put off the decision because of work: demanding schedules, business trips, and fear that colleagues will find out about the treatment. Coding at Narcology 24 is anonymous, with no registry, and usually takes a single visit after preparation. We see patients from all districts — Yesil, Saryarka, Almaty, Baikonur and Nura — and from the suburbs of Kosshy and Koyandy. Relatives can come to the first consultation together with the patient.

When a family should discuss coding with a doctor

  • your loved one says they want to quit but cannot manage without support
  • binges keep recurring despite promises and attempts to stop
  • the person has already been through detox and is ready to stay sober
  • work, health and family relationships are suffering because of alcohol
  • previous attempts to quit “on willpower” ended in relapse
  • the person is ready for a medical workup and a conversation with an addiction doctor
  • the family is ready to support sobriety, not just to control

If heavy drinking has lasted more than two or three days, or there is shaking, vomiting, blood-pressure spikes or insomnia, you need a doctor rather than stopping alone. Phone advice is free. For seizures, confusion or hallucinations call 103 or 112.

How coding works and why consent is required

Coding methods can be medication-based — for example, drugs that cause an unpleasant reaction when alcohol is consumed or that reduce cravings — or psychotherapeutic, based on suggestion and building a commitment to sobriety. All of them work as an “external brake” that helps the person hold on to their decision in difficult moments. But they do not change the reasons why the person drank, which is why psychotherapy matters too.

Coding without the person’s knowledge is impossible and dangerous: medication-based methods can cause a severe reaction if alcohol is consumed, so the patient must know the risks and sign the informed consent themselves. This is enshrined in the law of the Republic of Kazakhstan. Relatives can motivate, offer support and come to the consultation — but the decision is made by the person themselves.

How coding works: steps for the patient and the family

The first step is a consultation, including online for the family: the doctor explains the methods, answers questions and helps prepare the conversation with your loved one. Then the patient comes for an in-person appointment: a conversation with the addiction doctor, an examination and, if needed, tests and an ECG. A period of sobriety is required — the doctor determines its length depending on the method. Once the method is chosen and the consent form is signed, the procedure is carried out. The doctor explains the restrictions and stays in touch.

Coding is not carried out during intoxication or a binge, with acute illnesses, serious heart, liver or kidney disease, psychotic disorders or pregnancy, or without the patient’s consent. First, the binge must be ended and the period of sobriety maintained. If after coding the person drinks and feels unwell — chest pain, shortness of breath, loss of consciousness — call 103 or 112 immediately.

How we help

01

Medication-based coding

Drugs are used that cause an unpleasant reaction when alcohol is consumed, or medications that reduce cravings. They are given intravenously, intramuscularly or as an implant. The choice depends on the patient’s health and preferences. A medical workup, information about the risks and written consent are mandatory. The specific medication and period are prescribed by the doctor.

02

Psychotherapeutic methods

These include suggestion-based methods, among them the Dovzhenko method, and other forms of psychotherapy. Their essence is building a lasting commitment to sobriety through suggestion and motivational work. Effectiveness depends on the person’s readiness and on whether sobriety is supported by further work with a psychotherapist.

03

Preparation and medical workup

Before the procedure, the addiction doctor checks the heart, liver and blood pressure and orders tests if needed. Contraindications, current medications and the period of sobriety are discussed. It is important for relatives not to pressure or rush: “quick” coding right after a binge can be dangerous and gives worse results.

04

Support after coding

Coding is part of comprehensive treatment. After the procedure, Narcology 24 offers consultations with an addiction doctor and a psychotherapist, as well as work with the family. It is important for loved ones not to turn into “inspectors”: rather than checking every step, notice the effort, support the new sober lifestyle, and discuss difficulties openly and calmly.

Myths and facts

MythYou can code a person without their knowledge

FactNo. Coding is carried out only voluntarily, after informed consent. Secretly giving someone medications is life-threatening and against the law. The family’s task is to motivate, not to force.

MythCoding cures alcoholism forever

FactCoding helps maintain sobriety for a certain period but does not eliminate the addiction on its own. The best results come in combination with psychotherapy, family support and lifestyle changes.

MythAfter coding, a single drop of alcohol is fatal

FactThe consequences depend on the method. Medication-based methods can indeed cause a severe reaction if alcohol is consumed, so the doctor explains the risks in detail. Intimidation instead of information is unacceptable.

Questions and answers

How can I persuade my loved one to get coded?

Do not try to persuade them while they are drunk or during an argument. Choose a calm moment, talk about yourself and your feelings, and suggest going to a consultation together — with no obligation to get coded. A psychologist can help you prepare for this conversation. The decision must be the person’s own.

How long does one need to stay sober before coding?

The period of sobriety depends on the method and the person’s health: usually from several days or longer. The exact period is set by the doctor after a consultation. If the person is currently on a binge, detox is needed first, followed by preparation for coding.

For how long can a person be coded?

The period is discussed with the doctor: from several months to several years, depending on the method. It is better to start with a realistic period that the person is ready to keep to. It can be extended at the patient’s decision after a follow-up consultation.

Can relatives attend the consultation?

Yes, if the patient agrees. Your presence helps the doctor understand the situation better. The doctor may hold part of the conversation with the patient alone — this is normal and important for honesty and trust, not a way of hiding something from you.

What should we do if the person relapses after coding?

Do not panic or blame them. If chest pain, shortness of breath or severe weakness appear, call 103 or 112 immediately. If their condition is stable, call the doctor who did the coding. A relapse is a reason to adjust treatment, not to conclude that everything is useless.

Will anyone at work find out about the coding?

No. Treatment is provided anonymously on a paid basis, without registration with the state dispensary. The information is protected by medical confidentiality under the Code of the Republic of Kazakhstan “On Public Health and the Healthcare System” and is disclosed only in cases expressly provided for by law.
