12-step program in Astana

In short

The 12-step program is a structured path to recovery from addiction that began in the Alcoholics Anonymous fellowship and is now used for alcoholism, drug addiction, and gambling addiction, as well as in groups for relatives. At Narcology 24, elements of the program are part of rehab: counselors and a psychotherapist help people work through the steps, and families in Astana learn about groups for loved ones; the work usually continues for months and years.

What you should know

Many relatives have heard of the “12 steps” but have only a vague idea of what they are: some kind of sect, or perhaps a religious gathering. In reality, it is a mutual support program in which people with addiction support each other, admit the problem, take responsibility for their actions, and learn to live without the substance or gambling. The program does not replace treatment by a doctor, but for many people it becomes an important source of support for years after discharge.

In Astana, as in other large cities, there are mutual support groups for people with addiction and separate groups for relatives, including online ones. For those who have moved from other regions and haven’t yet found their own circle, the group often becomes the first place where they can speak honestly. We explain to the family how the program works, how it can help, and where medical and psychotherapeutic care is needed.

Which loved ones the 12-step program may suit

  • the person has completed detox and is looking for support to avoid relapse
  • feels lonely after treatment and has no one to talk to about cravings
  • their old social circle is tied to substance use, and they need a new community
  • being able to attend meetings for free and often, at any time, matters to them
  • they are willing to speak honestly about their addiction among peers
  • relatives are exhausted and looking for a place where they’ll be understood without explanations
  • long-term support is needed after the rehab course ends

If two or three points apply, it is worth seeing a doctor. Phone advice is free and confidential. If life is at risk, call 103 or 112.

How the program works and why it helps

The steps are sequential stages: admitting powerlessness over the addiction, being willing to accept help, an honest review of one’s life, admitting mistakes, making amends to loved ones, daily self-reflection, and helping others. The program is spiritual but not religious: each person understands the “higher power” in their own way, including as the support of the group. A mentor (sponsor) and regular meetings where participants share their experiences are important.

Research shows that active participation in mutual support groups is linked to longer-lasting sobriety, especially when combined with professional treatment. International guidelines recommend that doctors inform patients about such groups. For families, there are separate fellowships of relatives in which loved ones work on the same principles: they stop taking responsibility for someone else’s substance use and take care of themselves.

How the program is introduced as part of treatment

During rehab, an addiction counselor introduces the person to the steps, works through the first ones in individual and group sessions, and explains how to find a group and a sponsor. The psychotherapist helps connect the step work to personal difficulties. In a consultation, in person in Astana or online, the family is told about groups for relatives and how to support their loved one’s participation without turning it into a chore done “for show.”

A group does not replace medical care: binge drinking, withdrawal, psychosis, severe depression, or suicidal thoughts require a doctor first, and if there is a threat to life, call 103 or 112. If a person has a mental health condition and takes medication, it must not be stopped on the advice of group members. Online groups are convenient, but the first meeting with a counselor is better in person.

How we help

01

Step work with a counselor

In the rehab program, an addiction counselor — often someone with their own experience of recovery and professional training — guides the participant through the first steps: admitting the problem, being ready for change, and an honest review of the past. The work is done in writing, in conversations, and in groups. The pace is individual.

02

Group meetings

At meetings, participants tell their stories, listen to others, and receive support and hope. The rule is to speak about yourself, not to give advice, and not to judge. Regularity matters: in the first months, frequent attendance is recommended. There are in-person groups in different districts of the city and online meetings that suit busy schedules.

03

Combining with psychotherapy and treatment

We see the 12 steps as part of a comprehensive approach: medical care from an addiction doctor and a psychiatrist, cognitive behavioral therapy, family work, and mutual support groups complement each other. If the program doesn’t fit a person’s worldview, there are other group formats, and we will help find a suitable one.

04

Groups for relatives

Separate fellowships based on the same principles exist for the loved ones of people with addiction. Relatives learn to let go of control, take care of themselves, and stop blaming themselves for someone else’s illness. The specialists at Narcology 24 tell families about such groups and also offer individual consultations for the family in parallel.

Myths and facts

MythThe 12 steps are a sect

FactMutual support groups are open and free, keep no one against their will, and do not require anyone to give up their family, job, or beliefs. Each participant decides for themselves whether to attend meetings, how often, and for how long.

MythYou have to be religious

FactThe program speaks of a “higher power,” but each person understands it in their own way: as the group, as principles, or as their own sense of meaning. People with a variety of views take part in the fellowships, including non-believers.

MythGroups replace a doctor

FactGroups provide support, not medical treatment. Withdrawal, mental health conditions, and co-occurring illnesses require care from an addiction doctor and a psychiatrist. The best results come from combining treatment with community.

  4. A plan for the next daysAdvice for the family on what to do overnight and tomorrow, phone follow-up and a repeat visit if needed.

Where to get help

In astana you can get help at home, at one of our two branches (6 Auezova St., office 37, Astana, 176 Imanova St., Astana) or online if you need a consultation rather than a procedure. The dispatcher answers 24/7 and will tell you straight away which option is safest.

Confidential and private

We do not share it with employers, schools, the police or neighbours; the only exceptions are those expressly set by law. A first name is enough to call us, and paid confidential treatment does not mean a registry entry.

Questions and answers

Can relatives attend groups?

Yes, there are separate groups for the loved ones of people with addiction, including online. There you will meet people going through similar experiences and learn to take care of yourself. The specialists at Narcology 24 will tell you how to find them and offer individual consultations for the family.

How long should someone attend groups?

There is no fixed timeframe. In the first months of sobriety, frequent attendance is recommended; later, many people keep going regularly for years because it helps them stay sober and gives them a social circle. The decision always rests with the person themselves.

What if my loved one refuses to go to groups?

Don’t force them: pressure creates resistance. Suggest trying a few meetings of different groups, including online. If the format doesn’t suit them, there are other kinds of support — psychotherapy groups and individual therapy. The main thing is that support is in place.

Do I have to give my name at meetings?

No. Anonymity is a core principle of the fellowships: you can give just your first name or a made-up name. At our clinic, treatment is anonymous, with no registration on any registry, and all information is protected by medical confidentiality under the Code of the Republic of Kazakhstan “On Public Health and the Healthcare System”.

Can groups be combined with taking medication?

Yes. If a doctor has prescribed treatment for depression or anxiety, or medication to support sobriety, it must not be stopped on your own or on the advice of group members. Today’s fellowships generally understand the importance of medical treatment. Discuss any doubts with your doctor.

Where do we start if the person is still drinking or using?

If there is a binge or signs of withdrawal, medical help comes first — a home visit from an addiction doctor or inpatient care. In Astana in winter, it is especially dangerous if an intoxicated person ends up outdoors: the frost and wind are life-threatening. Once the person is stable, rehab and an introduction to the groups can begin.

Where do doctors visit in Astana and where are the branches?

Home visits cover all city districts and nearby settlements, 24/7. Appointments at two branches: 6 Auezova St., office 37, Astana and 176 Imanova St., Astana. Phone in Astana: +7 (700) 309-30-17.
